Our Case Management Team focuses on delivering personalized care to any member who needs extra assistance in improving their healthcare. From new to existing conditions the team will guide you through the system – coordinate appointments, address gaps in care, provide health education and conduct periodic telephone calls to help keep you healthy.

What is Case Management?
- Assist in well-coordinated care between your Primary Care Provider (PCP), specialists, imaging center and hospitals
- Assist with prescription cost savings
- Provide free patient education classes on various health-related topics
- Provide after hospital follow-up to make sure you are doing okay and have all of the necessary care, supplies and equipment
